An early authentic source in Jewish tradition narrates the custom of placing a stone on the grave that one visits. Located in a commentary on the Shulchan Aruch - the Code of Jewish Law - it explains that the custom of placing stones on the grave is for the honour of the departed. This symbolic act marks the fact that their grave has been visited and in turn has spiritual esoteric connotations.
